Uninvestigated Accident Report - Motorists may use this form to report a motor vehicle crash that was not investigated by the Greenfield Police Department.

Massachusetts Crash Report - State law requires a person who was operating a motor vehicle involved in a crash in which any person was killed or injured or in which there was damage in excess of $1,000 to complete and file a Crash Operator Report within five days following the crash.
